"I specifically searched for a clinic that performs both eye and endoscopic forehead lifts together, rather than a place that only focuses on eyelid surgeries."

This is exactly what a patient said as soon as she sat down for her anti-aging eye surgery consultation at our clinic recently.

Just a few years ago, if a patient came in for sagging eyelids (upper blepharoplasty) and I recommended an endoscopic forehead lift, they often misunderstood it as overtreatment, asking, "I came to fix my eyes, why are you telling me to do a forehead surgery?"

Nowadays, however, patients are highly educated. They understand the clear limitations of doing an upper blepharoplasty alone and actively request a combined approach. Today, I am going to clearly explain why so many top surgeons point to the "forehead" as the ultimate answer for sagging eyes, and the 5 reasons why a forehead lift has become the essential "foundation" of middle-aged eye surgery.

1. The Skin Thickness Dilemma (The Limits of Upper Blepharoplasty)

When people think of treating sagging eyelids, the first procedure that comes to mind is an upper blepharoplasty—cutting away the excess skin along the double eyelid line. However, there is a structural limit to how perfectly this can resolve sagging eyelids. The reason is "skin thickness."

Eyelid skin is divided into 3 zones based on thickness

The skin on our eyelids is thinnest and most delicate near the eyelashes, and it becomes progressively thicker and stiffer as it goes up toward the eyebrows. During consultations, I often compare the thin skin near the eyelashes to tissue paper, and the thicker skin near the eyebrows to thick cardboard.

If we simply cut away a large chunk of the thin skin near the double eyelid to fix the sagging, what happens? The surgeon is forced to stitch the remaining thick skin down to the thin area. This thick skin folds over itself, creating a puffy, unnatural, and heavy look commonly known as "sausage eyes."

An endoscopic forehead lift completely avoids this problem. It preserves the beautiful, thin skin around the eyes and resolves the sagging by lifting the droopy eyebrows and the entire forehead upward. This results in a much more natural, lighter, and refreshed eye shape.

2. "Opening the Eyes with the Forehead" (The Secret to an Unnatural Blepharoplasty Results)

If your pants are too long, you unconsciously walk on your tiptoes to keep them from dragging on the floor, right? Our eyes work the exact same way.

If there are saggy upper lids, the eyebrows are unconsciously lifted
(Change in eyebrow height when opening and closing the eyes - before surgery)

As we age and our eyelid skin sags, our brain subconsciously commands our forehead and eyebrow muscles to lift upward so our vision isn't blocked. It’s very similar to subconsciously standing on your "tiptoes" because of long pants.

But what happens if you undergo an upper blepharoplasty or a sub-brow lift that only removes the sagging skin? Because your field of vision is suddenly cleared, you no longer need to strain your forehead. The forehead muscles that were constantly tensed will completely relax, causing the eyebrows that were held high to instantly drop down.

💡 Does a lowered brow always mean a bad?

Not always. For patients who originally had a very large gap between their eyebrows and eyes (especially in men), this can actually have a positive effect, making the eyes look more defined.

However, generally speaking, when the distance between the eye and the eyebrow becomes too narrow, it creates negative changes to your impression:

  • Your impression may become tired, harsh, and even angry.
  • The glabella (space between eyebrows) narrows, worsening frown lines and transverse nasal wrinkles.

If people tell you,

"You look so angry after your eye surgery!"

it can never be considered a successful anti-aging procedure. An endoscopic forehead lift firmly holds the descending eyebrows upward, maintaining a soft, warm, and gentle impression.

3. The Core of Revision Surgery: Restoring Youthful "Proportions"

There are countless patients who had double eyelid surgery in their 20s or 30s and return to the clinic decades later for anti-aging eye surgery. Asians generally have thicker eyelids than Westerners and tend to open their eyes using their foreheads, so even just getting double eyelid surgery in their youth slightly narrows the distance between the eyebrows and the eyes.

Usually, a double eyelid surgery slightly narrows the distance between the eyebrow and the eye.

However, what is more important than double eyelids is facial proportions.

If a patient reaches their 50s or 60s and undergoes another upper blepharoplasty because their skin is sagging, what happens? The space between the eye and the eyebrow becomes endlessly narrow, resulting in a stuffy, artificial, and overly tight appearance.

In these cases, performing an endoscopic forehead lift beautifully restores the narrowed gap back to the golden ratio of their youth, creating a much more sophisticated and elegant result.

4. The Magical "Accordion Effect" for Sagging Wrinkles

The simple act of pulling the eyebrows and forehead upward has the effect of pulling taut the layers of crushed, sagging skin on the eyelids.

It is the exact same principle as stretching out a folded "accordion"—the wrinkles naturally flatten out. You don't always have to aggressively cut away skin with a scalpel; in many cases, simply applying upward tension naturally resolves the sagging eyelids.

5. Faster Recovery Than Double Eyelid Surgery

Because of the intimidating word "Lifting," many patients worry about the downtime. However, the reality is that an endoscopic brow lift does not involve any incisions around the eyes, making it look quite natural after just one week.

This is actually a major advantage over an upper blepharoplasty, where the eyelids are typically still quite swollen and bruised at the one-week mark.

Is a Perfect Upper Blepharoplasty Enough for Anti-Aging Eye Surgery?

Recently, a heartbreaking Caucasian patient visited my clinic and shed tears in my consultation room.

She had received a sub-brow lift at another clinic and was devastated by the results, coming to me for a second opinion.

Consultation with a patient suffering from a wrong surgical choice

When I closely examined the surgical site, the quality of the incision and suturing was excellent. I could tell immediately that the surgeon who operated on her was highly skilled. So why was she crying in front of me?

While the surgeon may have perfectly executed the mechanical act of cutting and sewing, they missed a profound question: "Was a sub-brow lift truly the best method for her?"

Generally, Caucasian facial anatomy features a closer distance between the eyes and eyebrows, and culturally, an upward-tilting, lifted tail of the eye is highly preferred.

If it were me, I would have recommended a Fox eye lift or a direct brow-lift to refreshingly lift the tail of the eye, rather than a procedure that narrows the eye-to-brow distance even further.

Because she received a surgery that completely clashed with her natural facial structure, her eyes and eyebrows became practically glued together. She blamed herself for choosing a surgeon who focused solely on the eyelids, rather than one who could see the bigger picture.

💡 The Insight to See Beyond "Sagging Eyelids"

Most patients who walk into plastic surgery office say, "My eyelids are sagging." However, an experienced surgeon should not just take the patient's word literally and perform a one-dimensional treatment of simply snipping away sagging skin.

A surgeon must be able to accurately predict and explain the gains (clearer vision) and the losses (a harsher impression) of cutting the eyelid skin. Furthermore, we must look for 3D causes—is the eyelid actually sagging, or does it just look like it's sagging because the orbital fat has depleted, creating a "sunken" hollow?(링크) We must discuss a master plan together.

Conclusion: A Forehead Lift is the "Foundation" of Anti-Aging Eye Surgery

A successful middle-aged eye surgery cannot be perfected by looking at the eyes alone. It requires an approach that understands the organic, interconnected movements of the entire upper face—from the forehead to the eyebrows, and down to the eyelids.

At Noonopi Plastic Surgery, we have accumulated data from over 500 diverse anti-aging eye surgeries, comprehensively considering the patient's eyebrow position, skin thickness, and forehead muscle usage. We have proven our expertise by publishing this precise, customized protocol in the world-renowned SCI-level medical journal, Springer.
