1. The Skin Thickness Dilemma (The Limits of Upper Blepharoplasty)
When people think of treating sagging eyelids, the first procedure that comes to mind is an upper blepharoplasty—cutting away the excess skin along the double eyelid line. However, there is a structural limit to how perfectly this can resolve sagging eyelids. The reason is "skin thickness."
The skin on our eyelids is thinnest and most delicate near the eyelashes, and it becomes progressively thicker and stiffer as it goes up toward the eyebrows. During consultations, I often compare the thin skin near the eyelashes to tissue paper, and the thicker skin near the eyebrows to thick cardboard.

If we simply cut away a large chunk of the thin skin near the double eyelid to fix the sagging, what happens? The surgeon is forced to stitch the remaining thick skin down to the thin area. This thick skin folds over itself, creating a puffy, unnatural, and heavy look commonly known as "sausage eyes."
